Not for most people — but the label is genuinely hiding something, and that's the real problem. Under EU cosmetics law a manufacturer may list an entire perfume composition as the single word "parfum," which can represent dozens of individual chemicals you never see. Fragrance allergy affects roughly 1% of adults and is second only to nickel as a cause of allergic contact dermatitis, according to DermNet. So if your skin is calm, fragrance is unlikely to be harming it. If your skin is reactive, "parfum" is the single most likely culprit on the label — and the hardest to investigate. The honest complication most brands skip: natural scent is not automatically safer, because many of the regulated allergens are naturally occurring plant molecules. What "parfum" legally means

Article 19(1) of EU Cosmetics Regulation 1223/2009 requires cosmetic products to list their ingredients on the packaging — with one significant carve-out. As the European Commission states, "perfume aromatic compositions and their raw materials are referred to as 'parfum' or 'aroma'." That single word is a trade-secret shield: it can cover a blend of dozens of aroma chemicals, and the consumer sees none of them.

There is one exception. Certain substances established as contact allergens must be named individually, no matter that they arrived inside a fragrance blend. At present 26 fragrance allergens are subject to that individual labelling requirement under Annex III. This is why you sometimes see "limonene" or "linalool" tacked onto the end of an ingredient list after "parfum" — those aren't separate additions, they're disclosures of what's inside the perfume.

The threshold matters too: individual labelling kicks in above 0.001% in leave-on products and 0.01% in rinse-off products. Below that, the allergen is present but invisible. As DermNet notes among the limits of investigating fragrance allergy, "ingredients must only be listed in a regulated product if they surpass a specified threshold concentration," and "manufacturers may not list any or all of their ingredients."

How common is fragrance allergy, really?

Worth keeping in proportion. DermNet puts fragrance allergy at around 1% of adults and about 1.8% in children and adolescents — common as contact allergies go, but not a majority experience. It is second only to nickel allergy as a cause of allergic contact dermatitis. Among people who actually undergo patch testing (a self-selecting group with existing skin problems), around 10% turn out to have a fragrance allergy.

The reaction is a delayed type IV hypersensitivity: it requires prior sensitisation, and it appears hours to days after contact, not immediately. That delay is exactly why people struggle to identify the culprit. It classically presents as scaly, itchy red patches, often in a streaky pattern following where the product was spread — most often the face, neck and hands in women, and the hands, face and lower legs in men.

One underappreciated route: the chemical can be carried from your hands to your face, so the rash may appear somewhere you never deliberately applied the product.

The information most articles leave out: "natural" is not "allergen-free"

This is where the internet's fragrance discourse falls apart. The clean-beauty version says synthetic fragrance is the villain and essential oils are the safe alternative. The dermatology evidence doesn't support that framing.

A European Scientific Committee on Consumer Safety review of fragrances in 2011 listed 82 substances established as contact allergens: 54 synthetic chemicals and 28 natural extracts. Twelve of the chemicals and eight of the natural extracts were flagged as high risk for sensitisation. Natural extracts are, proportionally, well represented among the problem ingredients — not exempt from them.

Look at the EU's own list of 26 and the point becomes obvious. Here is what those regulated allergens actually are, and where they come from:

Roughly two-thirds of that list is either a plant molecule or derived from one. A serum scented with lavender and sweet orange contains linalool and limonene as surely as a designer perfume does. DermNet's guidance to the fragrance-allergic patient is explicit on this: products "may contain an unidentified fragrance, for example, if they list 'botanical', 'herbal', or 'natural' ingredients."

The same page flags a second trap. "Fragrance-free" and "unscented" are not synonyms — DermNet notes that so-called unscented products "could still include masking fragrances," added specifically to neutralise the smell of the base. An unscented product can contain more fragrance chemistry than a scented one.

Essential oils deserve the same scrutiny

Because essential oils are heavily marketed as the natural alternative, it's worth being precise about them. DermNet's review of allergic contact dermatitis to essential oils notes that each essential oil can have over 100 constituents, and that people who react generally react to several oils rather than one — presumably because of those shared constituents. The oils most often implicated include tea tree, ylang-ylang, lemongrass, sandalwood, clove, jasmine absolute and narcissus absolute.

The most important practical point from that review: people should not apply neat, undiluted oils directly to the skin, because that is precisely how sensitisation happens. How to actually test a product on yourself

Rather than guessing from labels, dermatologists use a simple at-home protocol called the repeat open application test (ROAT). DermNet describes it as often more practical and cost-effective than formal patch testing for screening a new product. Here it is as a step-by-step:

1. Choose a test site — the inner forearm or inner upper arm. 2. Apply a dot of the leave-on product to that same spot twice a day. 3. Continue for two full weeks. Don't stop at day three; a type IV reaction can take days to build. 4. If a rash develops at any point, stop and don't use the product. 5. If nothing happens in two weeks, the product is very unlikely to be a problem for you.

Two caveats. This works for leave-on products, not undiluted wash-off products, which are irritating if left on skin regardless of allergy. And if you already suspect a fragrance allergy, formal patch testing through a dermatologist — using Fragrance Mix I, Fragrance Mix II and balsam of Peru — is the proper route. So should you avoid fragrance?

An honest, non-alarmist summary. If your skin is not reactive, fragrance in cosmetic concentrations is not doing you harm, and there is no good evidence it accelerates ageing or damages the barrier in normal use. If your skin is reactive — dermatitis, unexplained itching, eczema-prone, or a known fragrance allergy — fragrance is a rational first thing to eliminate, precisely because "parfum" makes it impossible to know what you're being exposed to. And if you're avoiding it, judge the ingredient list rather than the marketing: a product listing "parfum" plus limonene and linalool is being more transparent with you than one that says "naturally scented" and leaves it there.

The cleanest position, if you want one, isn't a fragrance-free product. It's a product with no perfume composition in it at all — where any scent is simply what the functional ingredients smell like.
